General Information

In order for the district office to keep our records up-to-date, please report any demographic/status changes to us immediately – name change, change of address, phone number, birth of child, etc. It is necessary to provide proper documentation for some changes such as a marriage license; a divorce decree; or a birth certificate and social security card for any child (not only newborn) to be added to benefits. Send completed forms to the District Office. Note: You only have 30 days to report a change in status occurrence to Human Resources in order to make changes to your benefits. The Health Savings Account and Waiver Plan explains this in further detail. If you fail to report the change in status occurrence within 30 days, you will not be able to make a change to your benefits until the next open enrollment period (May-June).
